This policy explains what information NextPhase Human Performance LLC collects through nextphasehp.com, why we collect it, who it is shared with, and the choices you have. We keep it short and specific to what this website actually does.
Who we are
NextPhase Human Performance LLC provides personal training and nutrition coaching in San Francisco and on the Peninsula. We are the controller of the information described here. You can reach us at [email protected] or (628) 333-4501.
What we collect
Information you give us
When you complete the assessment request form, we receive what you enter:

You may also email or call us directly, in which case we have whatever you choose to send.
Information collected automatically
Like most websites, ours records standard technical information through our hosting and advertising providers: IP address, approximate location derived from it, browser and device type, pages viewed, and the site or advert that referred you. We do not use this to identify you personally.
Health and training information
The form's open text fields invite you to mention injuries, surgeries, pregnancy or postpartum status, medications including GLP-1 treatments, and other health considerations. Share only what you are comfortable sharing — you can leave those fields blank and discuss them in person instead.
NextPhase Human Performance is a fitness and nutrition coaching business, not a healthcare provider, and is not a HIPAA covered entity. Information you send us is therefore not protected health information under HIPAA. We treat it as confidential and use it only to train you safely and appropriately, but you should know the distinction before deciding what to include.
How we use it
- To respond to your enquiry and arrange an assessment
- To design and adjust training and nutrition programs for you
- To keep records of our coaching relationship, including scheduling and payment
- To understand which adverts and pages bring people to us, in aggregate
- To meet legal, tax and insurance obligations
We do not use automated decision-making, and we do not build profiles about you beyond what is needed to coach you.
Who we share it with
We do not sell your personal information, and we do not share it for cross-context behavioural advertising. We never rent, trade or publish it. It is handled by a small number of service providers who process it on our behalf:

We may also disclose information if required by law, or to protect the safety and rights of clients, the public or the business.
Cookies and advertising
This site uses Google's advertising tag to measure the performance of our adverts — specifically, to record that an enquiry came from an advert click. That tag may set cookies in your browser. We do not use cookies to track you across unrelated websites, and we run no third-party advertising on this site.
You can block or delete cookies in your browser settings, and you can opt out of personalised Google advertising at myadcenter.google.com. Blocking cookies does not affect your ability to use this site or submit the form.
How long we keep it
- Enquiries that don't become clients: deleted within two years
- Client records: kept for the coaching relationship and up to seven years afterwards, which tax and insurance requirements call for
- Website and advertising analytics: retained according to the providers' own schedules
Your choices
You can ask us at any time to:
- Tell you what information we hold about you
- Correct anything inaccurate
- Delete your information, where we are not required to keep it
- Stop contacting you — by phone, text, email, or entirely
Email [email protected] and we will action it within 45 days. We will not treat you differently for exercising any of these rights.
California residents
Under the California Consumer Privacy Act, you have the rights listed above, plus the right to know what categories of personal information we collect and why, and the right to opt out of any sale or sharing of it. As stated, we do not sell or share personal information, so there is nothing to opt out of. You may designate an authorised agent to make a request for you.
Security
This site is served over HTTPS, and form submissions are encrypted in transit. Enquiries live in our Google Workspace email, protected by strong authentication. No system is perfectly secure, so please avoid sending anything highly sensitive — financial details, government ID numbers — through the form.
Children
We coach youth athletes, and their enquiries should come from a parent or guardian. We do not knowingly collect information directly from anyone under 13. If you believe a child has provided information through this site, email us and we will delete it.
Changes
If this policy changes we will update it here and revise the date at the top. Material changes affecting how we use information already collected will be communicated to current clients directly.
